Home Care

Continue to lead a quality, independent and dignified life within your own home

We believe getting older or having a disability shouldn’t mean losing your independence or dignity. Our home care, often called domiciliary care, enables older or less physically able people to continue to lead a quality, independent and dignified life within their own home.

Our home care support is tailored to your individual needs, from practical support with everyday living tasks, such as housework and shopping, through to help with personal care, meal preparation and feeding, and medical or nursing support.

Home care enables you to feel safe and comfortable within your own home. We also support you in maintaining your independent lifestyle outside the home, by accompanying you on outings, hospital appointments and using local services, such as the library or the shops.

Your care plan is individually designed in partnership with you and your key worker to provide you with the homecare and support you want and need, where and when you want it. It can be anything from a 15 minute check call, to 24 hour live in care.

Key workers

Our key worker system allows you to choose the key worker who will be the point of contact for you and your principal care/relatives. They’ll be available to answer any questions, support your daily care routine or just chat with you when you wish.

Initially your key worker will help you to note your preferred name, personal circumstances, personal history and social and medical circumstances. They will also assess your psychological and physical capabilities, as well as noting previous work and hobbies, likes and dislikes (when it comes to activities and food for example). Your personal programme of support and/or care can then be developed.

He/she will continue to support your ongoing needs, arranging Service User Plan meetings with you and your family or friends, and measuring the progress of your care programme.
